Before your appointment
Please fill out any and all forms that apply to you. Forms only need to be completed once per customer.
Policy Update Release
All clients (new and established) with an appointment after March 2020 must fill this out before your arrival to the salon before every future appointment. This document is updated as the needs and capacity of the salon change while the city moves in and out of Phases.
Color Consent Form
All clients who are receiving a color service must fill out before services can be performed
Gift Card Purchase Form
If you’d like to purchase a gift card please fill out the form below and submit it to us. We will email you shortly with your total and to confirm a pickup or delivery time.
